The book showcases the work of Trevor Kletz with new case studies and insights into latest practices. It covers the topics that he identified to process safety.
Rereading all of Trevor's books and other works reminded me how much I have been influenced by him in my career. I was genuinely surprised at how much of what he said I still apply today, almost instinctively. Unfortunately we are still having accidents around the world that perfectly illustrate what Trevor was saying decades ago.

Risk - Safety - Human Factors
Andy Brazier is a risk consultant specialising in human factors and process safety. He has a chemical engineering degree, PhD and vast consultancy experience. He has worked as a consultant since 1996 and started his own business in 2005. AB Risk Limited is Andy Brazier's consulting company. AB Risk has been accepted as a Registered Consultancy by the Institute of Ergonomics and Human Factors (IEHF).
Andy uses his combination of engineering background and consultancy skills to assist companies to manage their risks in a sensible and cost effective way. He works mainly in hazardous industries including the oil, gas, chemical, power and steel manufacture. With a few projects in other industries including air traffic control, food and drink, financial and waste.

My path to risk consultancy started at school with A Levels in maths, chemistry and physics. I went to Loughborough University (including a year in industry working at an oil refinery) where I obtained a degree in chemical engineering (graduated 1990), followed by a PhD at Edinburgh University. The title of my thesis was ‘Sources of data for use in human factors studies in the process industry.’
I started working as a consultant in 1996. First for a small human factors consultancy and then for the risk and safety department of a larger company. In 2005 I decided to go it alone, first as a sole trader, moving on to form AB Risk Limited in 2008.
I believe my combination of academic learning and practical experience; engineering and human factors knowledge makes me particularly able to assess the risks my clients have to manage. They seem to find my recommendations helpful and effective. Also, I seem particularly able at engaging with the workers at the 'sharp end' and facilitating multi-discipline workshops.
I am a Chartered Member of the Chartered Institute of Ergonomics and Human Factors (MCIEHF) and Associate Member of the Institution of Chemical Engineers (AIChemE). I am a member of the editorial panel for the Loss Prevention Bulletin and regularly review papers for the annual Ergonomics and Human Factors conference.
I have a very long list of past clients that include Shell, BP, Centrica, Scottish and Southern Energy, Flogas, Millennium Chemicals, Tata Steel, Corus, Ensus, Lucite, Ineos, Maersk, Petrofac, Siemens, Huntsman, Veolia, Pfizer, Dragon LNG, Novartis, ConocoPhilips, Dow Corning, Kemira, OMV, Qatar Gas, Petroleum Development Oman (PDO), Kuwait National Petroleum Company (KNPC), Abu Dhabi National Oil Company (ADNOC) and Saudi Aramco.
Although most of my clients are based in the UK, I have worked across Europe and the Middle East. My projects have been deployed from the Arctic circle through to Australia.
